BackgroundThe increased risk of developing attention-deficit hyperactivity disorder (ADHD) in extremely preterm infants is well-documented. Better understanding of perinatal risk factors, particularly those that are modifiable, can inform prevention efforts.MethodsWe examined data from the Extremely Low Gestational Age Newborns (ELGAN) Study. Participants were screened for ADHD at age 10 with the Child Symptom Inventory-4 (N = 734) and assessed at age 15 with a structured diagnostic interview (MINI-KID) to evaluate for the diagnosis of ADHD (N = 575). We studied associations (...) of pre-pregnancy maternal body mass index (BMI), pregestational and/or gestational diabetes, maternal smoking during pregnancy (MSDP), and hypertensive disorders of pregnancy (HDP) with 10-year and 15-year ADHD outcomes. Relative risks were calculated using Poisson regression models with robust error variance, adjusted for maternal age, maternal educational status, use of food stamps, public insurance status, marital status at birth, and family history of ADHD. We defined ADHD as a positive screen on the CSI-4 at age 10 and/or meeting DSM-5 criteria at age 15 on the MINI-KID. We evaluated the robustness of the associations to broadening or restricting the definition of ADHD. We limited the analysis to individuals with IQ ≥ 70 to decrease confounding by cognitive functioning. We evaluated interactions between maternal BMI and diabetes status. We assessed for mediation of risk increase by alterations in inflammatory or neurotrophic protein levels in the first week of life.ResultsElevated maternal BMI and maternal diabetes were each associated with a 55–65% increase in risk of ADHD, with evidence of both additive and multiplicative interactions between the two exposures. MSDP and HDP were not associated with the risk of ADHD outcomes. There was some evidence for association of ADHD outcomes with high levels of inflammatory proteins or moderate levels of neurotrophic proteins, but there was no evidence that these mediated the risk associated with maternal BMI or diabetes.ConclusionContrary to previous population-based studies, MSDP and HDP did not predict ADHD outcomes in this extremely preterm cohort, but elevated maternal pre-pregnancy BMI, maternal diabetes, and perinatal inflammatory markers were associated with increased risk of ADHD at age 10 and/or 15, with positive interaction between pre-pregnancy BMI and maternal diabetes. The solution of the outer multiplicity problem in the tensor product of U(3) irreducible representations (irreps) developed by Biedenharn et al.(1–7) and realized through the well-known Draayer-Akiyama (DA) computer code(8) is extended to the quantum algebra Uq(3). An analytic formula for special stretched Uq(3) Wigner coefficients, $$\left\langle {(\lambda _1 \mu _1 ) H_1 , (\lambda _2 \mu _2 ) \varepsilon _2 \Lambda _2 m_2 \left| { (\lambda _3 \mu _3 ) H_3 } \right.} \right\rangle _{\max }^q $$ is derived using (...) a projection operator method.(9–10) In this expression Hi denotes the highest weight vector of the (λiμi) irrep; the subscript “max” means coefficients corresponding to a unit tensor operator with a maximal characteristic null space; and q is the usual quantum label so the standard U(3) Wigner coefficient, which is required in the DA code, can be obtained in the q→1 limit of the theory. To illustrate the theory, some Uq(3) Wigner coefficients for the tensor product (22)×(22) are calculated. Scholars of Plato are divided between those who emphasize the literature of the dialogues and those who emphasize the argument of the dialogues, and between those who see a development in the thought of the dialogues and those who do not. In this important book Russell Dancy focuses on the arguments and defends a developmental picture. He explains the Theory of Forms of the Phaedo and Symposium as an outgrowth of the quest for definitions canvassed in the Socratic dialogues, by (...) constructing a Theory of Definition for the Socratic dialogues based on the refutations of definitions in those dialogues, and showing how that theory is mirrored in the Theory of Forms. His discussion, notable for both its clarity and its meticulous scholarship, ranges in detail over a number of Plato's early and middle dialogues, and will be of interest to readers in Plato studies and in ancient philosophy more generally. We argue that thoughts are structures of concepts, and that concepts should be individuated by their origins, rather than in terms of their semantic or epistemic properties. Many features of cognition turn on the vehicles of content, thoughts, rather than on the nature of the contents they express. Originalism makes concepts available to explain, with no threat of circularity, puzzling cases concerning thought. In this paper, we mention Hesperus/Phosphorus puzzles, the Evans-Perry example of the ship seen through different windows, and (...) Few residency training programmes explicitly require substantive exposure to issues in medical ethics and fewer still have a formal curriculum in this area. Traditional undergraduate medical ethics courses teach preclinical students to identify ethical issues and analyse them at a theoretical level. Residency training, however, is the ideal time to establish the critical behavioural link which makes ethics truly useful in clinical medicine. The General Internal Medicine Residency Training Program at Rhode Island Hospital has developed an integrated, three-year curriculum with (...) the goals of helping residents to perceive ethical issues in clinical practice, to utilise basic philosophical principles in resolving ethical dilemmas and to communicate these issues clearly and sensitively to patients. The curriculum has been well received by residents and has had a hospital-wide impact. We believe that training residents in medical ethics and communication skills is an effective approach to developing physicians' humane qualities. (shrink)

I had a strange dream, or half-waking vision, not long ago. I found myself at the top of a mountain in the mist, feeling very pleased with myself, not just for having climbed the mountain, but for having achieved my life's ambition, to find a way of answering moral questions rationally. But as I was preening myself on this achievement, the mist began to clear, and I saw that I was surrounded on the mountain top by the graves of all (...) those other philosophers, great and small, who had had the same ambition, and thought they had achieved it. And I have come to see, reflecting on my dream, that, ever since, the hard-working philosophical worms have been nibbling away at their systems and showing that the achievement was an illusion. True, their skeletons, indigestible to worms, remained, and were surprisingly similar to one another. But I was led to think again about what one can, and what one cannot, achieve in this direction. Long before the invention of the mechanical clock, the monastic computes offered a model of time that was visible, durable, portable and objectifiable. The development of ‘temporal literacy’ among the Anglo-Saxons involved not only the measurement of time but also the ways in which the technologies used to measure and record time — from sundials and church bells to calendars and chronicles — worked to create and reorder cultural capital, and add new scope and range to the life of the (...) imagination. Techniques of time measurement are deeply implicated in historical consciousness and the assertion of identity; this paper proposes some avenues of exploration for this topic among the Anglo-Saxons. In attempting to date the Shield several complementary methods are possible. Roughly these may be classed as literary, historical and archaeological. The literary method indicates that the Shield comes late in the Hesiodic corpus: in particular the use of the F is careful. The historical method suggests a preciser upper limit. Wilamowitz believed that the point of lines 393–401, which give the season in which the combat between Herakles and Kyknos took place, can only be that a commemorative festival was (...) held—at the temple of Apollo at Pagasae, since the scene is laid in Apollo's sanctuary. Mazon continues that the reputation of the temple dates from the end of the Sacred War and the establishment at Pherae of a dynasty with close connections with Delphi : he therefore sets the terminus post quern at about 590. This argument is attractive and possible. Several scholars have attempted to secure a lower limit from the last sentence in Hypothesis A: κα⋯ Στησ⋯χορος δ⋯ φησιν ' Hσ7iota;⋯δου εἶναι τ⋯ πο⋯ημα. But as Wilamowitz observes, we do not know whether the poem in which this mention came was a genuine work of Stesichoros. Certainly it need not make the Shield much earlier than the Stesichorean poem, since our ‘Hesiod’ wrote his epyllion for incorporation in the Ehoiai which was attributed to the genuine Hesiod, and an insertion into such a work could have been very quickly accepted. Mazon's lower limit of 560 is then on insecure grounds. Thirdly there is the possibility that analysis of the decoration of Herakles' shield might give useful comparisons to the conclusions of archaeologists on the chronology of Greek art.
